When you click on "Appeal NTRP Rating," the form should open in Adobe Reader. When the form appears, you can print it and fill it out by hand. Deadlines and other pertinent information appears on the form itself.

Click on the one you want to download. From there you can print it blank, or you can enter your lineup with your computer, while the form is still visible on the monitor. Click on the area where you want to enter the first information, then hit "Tab" to move to the next field. Be sure to move off the last name you enter so that it will print.
ATTENTION Captains: You are welcome to use these Scorecards, but you may find it convenient to have the Score Entry Form that lists all rostered players and their ratings. To get that form, do the following: Go to national.usta.com and enter the number of the match you are about to play in the field at the bottom of the page. Click "Go" and then hit the button at the bottom titled "Print Blank Scorecard." The scorecard that comes up on your screen will have the rostered players listed at the bottom with their current ratings. You can print out the form from that screenIf any player should be on your roster, but is not, please make sure we know about it. If the reason is a problem we are unable to rectify prior to the match you should notify the opposing captain at the time of the match. The same is true if an incorrect rating is displayed on the print out. We will do what we can to correct any such problems as soon as possible. Just make sure that we (The League Coordinators) and the opposing captain know about it before the match.
